Schedule Your Easy as 1–2–3! Eczema Education Encounter with Dr. K
- Take the time to carefully read and understand the explanation and disclaimers for these educational sessions and make our very protective attorney happy.
- Scroll down towards the bottom of the page and find the section that looks like a calendar.
- Click a date and available time that works for you.
- You will be asked to enter the following information: First Name, Last Name, Phone Number, and Email Address.
- First Name and Last Name should be of the person for whom the appointment is being made (in other words, the person Dr. K is actually meeting with). Usually, this person is you, but it might be for a family member or friend.
- If the person for whom this appointment is being scheduled is less than 18 years of age (i.e., a legal minor) then the parent/legal guardian should enter his/her own name instead of the child’s.
- Please make sure to enter the correct Phone Number of the specific phone through which Dr. K will reach you using his HIPAA-compliant video link on the scheduled date and time of the educational session. Usually, this is a smartphone or similar device with audio/video and internet-connection capabilities. A landline will not work. If the phone number or internet-connection does not work or the person scheduled for the appointment has a different phone, then the educational session cannot take place, and the Gift Code may be forfeited.
- If you agree to the explanations and disclaimers around these sessions, then please check the corresponding box. You cannot be seen by Dr. K until you agree to these conditions.
- Answer – to the best of your ability! – the “Easy as 1-2-3! Eczema Education Encounter Intake” questions. These will give Dr. K a better idea of what you and he are dealing with.
- The first two questions are the only REQUIRED ones.
- If you are uncomfortable answering any other questions or do not know the answers, then skip them.
- Yes, Dr. K actually reads these intake forms quite carefully!
- Hit the “Continue to Payment” button.
- The “Payment Information” screen appears.
- If you are using a Gift Code, then find the section called “Package, gift, or coupon code” and click the “+” sign. An “Enter Code” field appears. Enter your Gift Code here and then hit the “APPLY” button.
- If you are paying for the visit, you may choose credit or debit card and enter the required fields to complete the purchase. The cash price of the appointment is $225.
- Hit the “Pay & Confirm” button.
- You will be brought to the “Appointment Confirmed” screen. Here you can do any of the following:
- Confirm the scheduled date and time of your educational session
- Add the scheduled session to your iCal/Outlook or Google calendars
- Edit your information including your eczema intake form questions
- Reschedule your appointment to a different date/time
- Cancel your appointment. If you used a Gift Code and cancel, your Gift Code should still be active for use later (unless it expires).
- At the date and time of your scheduled session, you will get a text message that says: “Your Telemedicine visit has started! Please tap here to join. Dr. Krakowski is on the call.” Click this message and you and Dr. K will be connected!
